Eyelid surgery is a procedure to remove fat--usually along with excess skin and muscle from the upper and lower eyelids. It can correct drooping upper lids and puffy bags below your eyes. However, it won't remove crow's feet or other wrinkles. While it can add an upper eyelid crease to Asian eyes, it will not erase evidence of your ethnic or racial heritage.

Many Asian patients without upper eyelid creases want to undergo the “double eyelid operation” because the eyelid creases will make their eyes look bigger and make it easier for them to apply eye makeup. Many of these patients know exactly what they want by bringing in photographs of their favorite movie stars and often, requesting fat removal from their eyelids and orbits. In fact, this rather common procedure can easily be performed under local anesthesia in an office setting in 30 to 40 minutes’ time. Sedation or anesthesia is generally unnecessary.

Dr. Eng's general advice to patients who want to have certain ‘movie star’ eyes is to remind them that the basic anatomy between the patient and the movie star may be too different, and that it may be impossible to produce the results they wish for. Patients who want ‘a lot of fat’ removed, should be aware that they may mistake the heavy eyelid muscles for fat. Muscle trimming will not always give these patients the results they are looking for. Orbital fat can be trimmed if excess fat is present. However, aggressive fat removal in young age aggravates the hollow orbit, which inevitably develops during old age. Furthermore, hollow orbits do not look good in Asian faces.
